Columbine Caregivers has been a leading provider of home healthcare services in the grand valley for over 30 years. We are currently looking for a great candidate to join our team in the role of Care Coordinator.
Primary responsibilities for the Care Coordinator role include:
Working with both clients and caregivers to ensure all aspects of care are managed appropriately. Care Coordinators meet with clients to set up initial care/service plans, work closing with scheduling to ensure schedules are then created to meet customer needs and provide on-going communication and support for both clients and field employees.
All supervisors participate in an on-call rotation. Must be comfortable with after hours and weekend on-call duties. Most of the on-call duties are carried out over the phone, however as a last resort, occasionally supervisors do cover shifts, to ensure clients have coverage for every shift.
Care Coordinators also assist with on-boarding new care staff through recruiting, interviewing caregiver candidates, facilitating new-employee orientations, and providing on-going communication and support.
Other Care Coordinator duties include, but are not limited to:
· Provide outstanding customer service both over the phone as well as in person
· General phone support
· Client file completion and data entry into EMR system
· Client and caregiver supervisory calls and visits
· New employee file completion and data entry into EMR system
· Communicate effectively and have the ability to problem solve
· Provide occasional back up support to cover caregiver shifts in time sensitive situations (when a caregiver calls out and a replacement cannot be found in time)
·Interagency communication
· All aspects of administration relevant to a small to mid-sized office team
· Assist with scheduling as needed
· Assist with general office tasks, including filing, maintaining office cleanliness, etc.
Office hours are Monday-Friday 8:30 – 4:00 but often times staying past 4:00 is necessary to finish working through time-sensitive scheduling needs. Typically Care Coordinators work 35-50 hours a week. Participating in the on-call rotation typically includes managing the afterhours phone 1 to 2 nights a week and 1 to 2 weekends a month. On-call responsibilities include helping with after-hours scheduling needs that arise. If a caregiver calls out for a shift the on-call supervisor makes the appropriate phone calls/texts to get another caregiver lined up to cover the shift and provides communication with clients regarding these schedule changes. Occasionally all supervisors will cover caregiver shifts, typically this is the last resort, however if a caregiver cannot be found to cover a shift the supervisor on call is expected to cover the shift to ensure clients are taken care of.
The ideal candidate will have:
-Experience working in an office environment
-Experience in home care, or other healthcare experience (Preferred, not required)
-Experience in a supervisory role (Preferred, not required)
-Experience in scheduling (Preferred, not required)
-A current CNA license (Preferred, not required)
Candidate will be required to pass an extensive criminal background check as well as drug testing prior to employment. We provide a company laptop.
