Impact Behavioral Health Partners, a mental health agency located in Evanston, seeks a qualified and experienced Clinical Case Manager to serve as a member of a multidisciplinary team.
As a Clinical Case Manager, you will provide a range of strength-based, person-centered services to Impact’s participants to promote their recovery, independence, and community integration.
This position is suited for an independent self-starter. Someone who can work flexibly in a hybrid work setting, providing services both remotely and in person, is more likely to do well. Excellent communication and organizational skills are also needed for this role. The Clinical Case Manager reports to the Clinical Supervisor.
Salary: Starts at $42,400 per year, bonus pay is possible.
Benefits: This position comes with a full benefits package. The vacation benefit package includes 25 days of PTO, 13 paid holidays, and 2 paid floating holidays each year. Employees who work at Impact at least one year are eligible for paid parental leave.
The insurance benefit package includes HMO or PPO health insurance through Blue Cross Blue Shield provided at no monthly premium cost to the employee for plans that do not include a spouse or family. Each employee also receives $10,000 life insurance coverage at no cost to them. Employees may choose to purchase additional life insurance, vision insurance, or dental insurance at affordable monthly costs.
The retirement package includes a 401(k) plan through Principal. Impact contributes 1.5% the amount of an employee’s salary to their 401(k), at no cost to the employee. Impact also pays the administrative fee charged by Principal for the 401(k).
Location: This role will be primarily on site between eight assigned locations in northern suburban Cook County, depending on need. Utilizing other work settings such as working remotely from home, working remotely in the field, or working at Impact’s office in Evanston will be possible as conditions and schedule allows.
Schedule: Full-time position, 37.5 hours per week. Flexible scheduling may be available to accommodate employee needs outside of work but cannot be guaranteed.
Essential Functions:
● Provide direct service to program participants in alignment with the clinical team’s case management strategies that are developed for each participant.
● Independently manage a caseload of 25 to 35 individuals.
● Provide a range of services that include but are not limited to case management, resource connection, team-based case coordination, crisis intervention, skills training in activities of daily living, and discharge planning.
● Assist participants in applying for and maintaining public benefits such as Medicaid, SNAP, Social Security, etc.
● Activities will most often occur in natural community settings, including on site at Impact’s residential locations.
● Conduct outreach and engagement activities such as presentations, door knocking, cold calling etc.
● Build and maintain relationships with partner organizations and community agencies through outreach and engagement activities.
● Fully promote and participate in the integration of Housing First, harm reduction, and trauma-informed care approaches across all services.
● Complete documentation of all relevant participant activities, such as progress notes, initial and on-going assessments (IM+CANS, LOCUS), crisis assessment/safety plans, critical incident reports, etc. in a timely manner.
● Meet all established caseload and billing requirements.
● Be available and provide coverage to the staff and program participants on an on-call basis after regular business hours as needed and assigned.
Additional Job Functions:
● Employees are required to meet agency standards in their personal vehicles for milage reimbursement. This includes but is not limited to employees personally obtaining and maintaining $100,000 /$300,000 bodily injury and liability auto insurance policy. Proof of insurance must be provided at least annually, and a department of motor vehicles check will be run on all employees’ driver’s license at least annually.
● Comply with all agency policies and procedures. This includes but is not limited to remaining familiar with and following all agency, CARF, Office the Inspector General, Medicaid, Medicare, state, and federal guidelines, as well as conducting safety drills, preparing reports as required, and participating on any assigned committees.
● Occasionally be available to work flexible schedules. In some instances, this is required as an accommodation for participants with special needs. In other instances, flexible scheduling may be necessary to assure adequate clinical team coverage.
● As needed, provide participants with hands-on training and education on how to clean, cook, do laundry, shop, maintain an apartment, and manage other activities of daily living. This may require lifting and carrying of small items.

Qualifications:
● Bachelor’s Degree in social work, psychology, sociology, or related human service profession.
● Demonstrated social services experience with persons with a chronic mental illness, preferably in a multi-disciplinary setting. One to two years relevant experience preferred, internships or employment.
● Demonstrated experience in and commitment to providing services from a harm reduction approach.
● Demonstrated ability to provide clear and effective communication both verbal and written.
● Ability to function in a computer-oriented office, including working knowledge of pertinent software. Must have access to private office area and reliable internet access while working remotely.
● Skills and abilities necessary to provide the responsibilities and requirements outlined above.
● Spanish speaking preferred
