Job Summary:
The Care Manager for this position would provide supportive services to an average caseload of 30 children who meet eligibility of NYS Children’s Health Home program. The Care Manager guides program enrollees and their caretakers (legal guardians) through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes. The Care Manager would work with children, their families and service providers to create a comprehensive plan of care, identify additional service providers as necessary and coordinates the continuing involvement of families and service providers in executing the plan of care. The Care Manager would also need to be an independent worker and be able to navigate (or learn to navigate) several electronic databases. Astor is looking for someone with experience doing home visits and making community referrals. The candidate could be from a homeless shelter, prevention program, or visiting nurses type program.
