Position Overview:
The primary task of the FCAP Case Worker is to assist each client and their family while they are in FCAP. The FCAP Case Worker is the case manager responsible for coordinating and monitoring services for clients and is the liaison to all community resources as they relate to discharge and family reunification. Each caseworker will do initial assessments and enter related data in the JDAS system. As a core member of the team the Case Worker engages with other disciplines such as community based agencies, school personnel, Family Court, Probation, OCFS, etc. Caseworker is additionally responsible to provide targeted interventions and collaborates with FCAP Specialists on special projects developed based on the skill set of this individual.
Qualifications:
Bachelor’s degree in Social Work or related field. Experience in counseling or human service area is preferred. Must possess exceptional time management and detail-orientation skills, and have the ability to manage multiple tasks. Must possess a valid state driver’s license. Spanish speaking preferred
