ICMS - Case Manager

Role Overview:
As a Case Manager - ICMS, your primary responsibility is to provide daily case management support to your assigned caseload. You'll build strong, trusting relationships with often extremely vulnerable participants, using a person-centered approach that includes motivational interviewing and harm reduction. Your role is pivotal in helping individuals take steps forward toward a brighter future.
Full-time (Monday to Friday)
$25.54 per hour
Key Responsibilities:
- Develop meaningful, trust-based relationships with vulnerable participants, promoting their holistic wellness and connection to essential services.
- Serve as the primary point of contact for clients, managing a caseload of no more than 20 individuals.
- Provide a range of ICMS case management services, regularly checking on clients' well-being and connecting them to necessary resources.
- Offer guidance, support, and opportunities to our staff, emphasizing harm reduction and trauma-informed care.
- Develop and facilitate case management services tailored to vulnerable populations.
- Perform essential administrative tasks, including attendance tracking and reporting.
- Identify service gaps and collaborate with community partners to ensure participants receive the full scope of necessary services.
- Collaborate with internal and external partners to facilitate participant care and advancement, navigating them through a variety of services in the Hollywood area.
- Attend staff meetings, training sessions, and retreats to ensure team synergy and stay updated on changes.
- Maintain effective communication with management, staff, collaborators, and participants.
Additional Responsibilities:
- Conduct tours as needed or requested.
- Attend seminars, conferences, and professional development training as required.
- Perform other duties as requested.
Requirements:
- Some experience in providing case management services to vulnerable populations.
- Any equivalent combination of education, experience, or training that has prepared you for the role.
- Skills and Abilities:
- Proficiency in computer applications, including word processing, spreadsheets, and internet research.
- Strong organizational and planning skills.
- Excellent interpersonal and communication skills for building relationships.
- Strong record-keeping skills for tracking metrics and participation.
Licenses & Certifications:
- California driver's license or equivalent.
Special Requirements:
- Successfully pass a background check.
Working Conditions:
This position may involve walking, climbing stairs, and occasional lifting (up to 20 pounds). You'll work both indoors and outdoors, often in various weather conditions, and in a moderate noise level office setting.
