A Clinical Manager is responsible for ensuring the patient care is coordinated and managed appropriately. The Clinical Manager will also ensure that care and services are delivered appropriately, as well as the supervision of clinical personnel.
Responsibilities:
- Receives case referrals and assigns appropriate clinician to case.
- Review patient's clinical diagnosis, medications, procedures, and clinical course.
- Assists in clinicians in establishing immediate and long term therapeutic goals, in setting priorities, and in developing plan of care.
- Conducts quarterly record reviews and communicates findings and recommendations to Clinical Director and appropriate organization personnel.
- Assists Clinical Director in the planning, implantation, and evaluation of in-service and continuing education programs.
- Attends meetings, CEUs on a regular basis
Working for a home health care agency requires the following:
- Registered Nurse with current licensure to practice professional nursing in the state.
- Graduate of National League for Nursing accredited school of nursing. BSN degree is preferred.
- Two years management or supervisory experience in health care setting, preferably home care.
- Complies with accepted professional standards and practices.
- Compiles and maintains current CPR certification.
- Demonstrates excellent observation, verbal and written communication.
- Computer skills; must be able to work with EMR system.
