Position Summary:
The Behavioral Health Intake Specialist will serve as the primary person to provide patient intakes for the Behavioral Health Team
Essential Functions (duties):
- As a staff member of the Community Based Wellness team, the Intake Specialist develops the initial integrated comprehensive assessment and Integrated Service Plan (ISP) for new service recipients.
- Completes intake packet, including, but not limited to: consents, notice of privacy practices, releases of information, etc.
- May be required to assist clinical team with routine, yearly comprehensive assessment and Integrated Service Plan (ISP) updates/ completion in order to ensure contact compliance expectations are met.
- Provides all new referral information to Integrated Case Manager and care team for their ongoing care.
- Coordinates with all involved healthcare, behavioral health and wellness providers and maintains current documentation.
- Serves as the initial point of contact for new service recipients via telephone or face-to-face interactions with a person, family, or other involved party for the purpose of facilitating new enrollment.

Coordinates and communicates with Clinical Manager, care team, and psychiatric practitioners for individuals exhibiting urgent or emergent medical and/or psychiatric symptoms.
- Consistently meet organizational encounter and performance goals and follow clinical documentation standards as defined by company policy

Act as a consultant to team members in order to support therapeutic progress of the clients in treatment.

Participate on Child and Family Teams for assigned clients.
- Provide coverage during Open Access as needed.
- Provide on call coverage as needed

Assist in actively assessing work area for non-compliance issues and notify supervisor in a timely manner to ensure follow up.

Minimum Education and Experience:
- Minimum bachelor's degree in counseling, psychology, social work, or related field of study.

Must have DSM-5 knowledge
- Preferred license by the Arizona Board of Behavioral Health Examiners, or license pending.
Prerequisites:

Must be 21 years of age.

Possess an Arizona driver's license and state mandated auto insurance.

Possess or be able to obtain a Level One Fingerprint Clearance Card.

Must be able to pass a 5-panel drug screen.
