Best Health Care Services is seeking for a new LPN/ licensed or limited experienced in Home Health Care , for daily patient skilled visits for our growing home health services. BHCS will train.
- The LPN treats and supervises patients health condition by providing quality of care designed for each patient in the comfort of their home under the supervision of RN with physician's orders and assigned Care Plan.
- The LPN is responsible for adequate clinical utilization to meet current and future demand for services. The LPN is crucial to successful care coordination with case management to provide quality of care to patients.
- Understanding of OASIS EMR is a plus for the skilled department.
- Medication mgt , lab draws is a plus.
- Virginia CCC Plus (optional) waiver documentation knowledge is a plus.
- Kinnser (WellSky) software knowledge is a plus. Kinnser software is user-friendly.
- Individual must have a current VA state LPN license.
- Must have excellent observation, verbal & written communication skills.
- Ability to independently manage time and organize daily schedule to meet productivity standards.
