Job Description:
Nursing and Rehab Center is seeking a highly motivated and experienced MDS Coordinator to join our team at a nursing home facility. The MDS Coordinator will be responsible for coordinating the development and completion of the MDS assessments for all residents in accordance with state and federal regulations. The successful candidate will work closely with interdisciplinary team members to ensure accuracy and completeness of all resident assessments, and to develop and implement individualized care plans that meet the needs of each resident.
Responsibilities:
- Participate in the admission process of prospective residents in terms of their nursing needs and appropriate placement.
- Determine potential Resource Utilization Groups (RUGs) and expense associated with a potential admission Review all applications for admission, followed by on-site or phone assessment and communicate results of assessment to the MDS Coordinator
- Complete and assure the accuracy of the MDS process for all residents
- Maintain current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicate changes in regulations.
- Monitor Case Mix Index (CMI) scores, looking for potential risks and/or changes that may affect Medicaid reimbursement Facilitate problem-solving for complicated admissions
- Monitor Medicare assessment schedules and nursing documentation to ensure accuracy and timely submission
- Collaborate with peers, physicians and other members of the team.
- Assess prospective admissions.
Requirements:
- Bachelor's Degree in Nursing (Valid NYS License Required)
- 1 years of clinical experience with a focus on MDS or Care planning

Pay: From $120,000.00 per year
