Qualifications:

Responsibilities:
- Coordinate/Supervise clinical operations
- Hire, Orient, and Train new clinical staff
- Ongoing evaluation of clinical staff, staff education, staff support
- Evaluate the performance of clinicians
- Performs skill pass-offs and competency evaluations for all clinical staff
- Maintain compliance with federal/state regulations as they relate to providing patient home visits
- Modification and development of policies and procedures as required
- Understands, investigates, and resolves patient complaints and concerns
- Displays thorough knowledge, understanding and ability to perform the operations a Home Health Agency, including: Intake, Patient Assessments and Reassessments, Nursing processes, Plan of Care, Discharge Planning, OASIS Data Collection
- Has a firm knowledge and understanding and keeps current with the evolution of the accrediting organization standards, federal, state and local regulations, OASIS, and Medicare requirements for home care reimbursement
- Assures quality care by reviewing and assessing each service and through client visits and other measures, as appropriate
- Competence in general nursing theory, techniques and practice and delivery of patient care that emphasizes patient safety and minimizes the likelihood of medical and health care errors
- Demonstrates competence in reporting victims of abuse, National Patient Safety Goals, reporting of medical and health care errors according to established facility standards and state and local laws
- Coordinates patient referrals
- Assures the development, implementation, and updates of the plan of care
- Assures clinical records are in compliance with local, state and federal laws.
- Ensure coordination of care by all disciplines
- Participate in review, analysis, and appraisal of the effectiveness of the complete agency programs
- Actively contribute to all short- and long-term planning for the agency and implement necessary measures for agency growth and increasing market share through education of physicians and other community and referral sources
- Consistently involved in state, local, and national organization, meetings, seminars, and activities relating to the home health & hospice profession and health care services
- Ensures timely and complete patient documentation, as applicable, and ensure all direct reports complete their documentation on time
- Initiates and manages staff meetings as needed
- Completes assigned training and continuing education on time as required
- Ensures and maintains patient confidentiality at all times
- Demonstrate knowledge and skills necessary to provide care to the geriatric and adult population
- Reviews all physician orders for accuracy
- Audit charts for clinical accuracy
- Governs the hospice program including patient care, volunteer, and bereavement programs
- Supervises infection control policies
- Assist with writing letters for insurance appeals regarding clinical content in conjunction with laws, regulations and conditions of participation
- Ensure company is abiding by all conditions of participation with Medicare
- Prepare for, plan and hold Interdisciplinary Group Meetings according to COP’s
- Ensures timeliness of 485’s signed by physicians within required timelines (home care)
- Ensures all criteria required in a plan of care is complete (home care)
- Ensures certification of terminal illness signed by physician within proper time frame (hospice)
- Track and complete hospice discharges
- Light Medical Coding
- CAHPS satisfaction monitoring coordination
- Manage Casper/Rand reporting
- Other duties as assigned

Pay: $90,041.00 - $97,835.00 per year
