Patient Engagement Specialist III (Population Health)
Job Purpose:
The Patient Engagement Specialist is responsible for contacting patients, conducting assessments, accessing, and then updating an electronic medical record (EMR), and scheduling patient appointments.
Main Duties and Responsibilities:

Review hospital discharge rosters and create list of patients to be contacted.

Contact patients (phone/email/text/mail) to schedule appointments within department guidelines

Utilize patient assessments to determine initial care needs and barriers to service.

Behavioral Health Tech (BHT) qualified only – Complete intake assessments and provide diagnosis.

Use motivational interviewing skills to engage patients.

Demonstrate empathy and compassion in all patient interactions.

Create, update and access confidential patient information while adhering to HIPAA guidelines.

Perform routine data entry into various software systems.

Add information to appropriate software resource.

Schedule appointments within specified guidelines

Meet (or exceed) department metrics for outreach and scheduling activities, assisting in the preparation of reports for tracking purposes; Tracking and providing patient related data as needed.

Other duties and special projects as assigned.
Requirements:

Non BHT: GED or HS Diploma (required); Medical Assistant or AA degree in healthrelated field preferred.
- BHT: Associate degree in nursing, LPN, counseling, psychology, social work, or related field preferred
- One (1) or more years’ professional experience in a similar role (customer service, marketing, referrals, billing, etc.); Experience in health care is highly desired.
