In-Home Individual Skills and Socialization Program

MINIMUM QUALIFICATIONS:
Must have a high school diploma or equivalent. Must meet requirements of HCS Certification Principles. Must be able to provide transportation in accordance with state laws.
PURPOSE:
This position provides day habilitation services to individuals enrolled in Assured Quality Care Services, LLC. Home and Community Based Services Program according to HCS Certification Principles and Billing Guidelines as implemented by the Health and Human Service Commission. This position provides the basic daily care that is required by the individual. In-home individualized skills and socialization is the provision of assistance to an individual that is necessary for the individual to acquire skills to reside, integrate and participate successfully in the community. In-home individualized skills and socialization must be provided in the individual’s residence.
ISS SERVICES DEFINITION INCLUDES:
(1) interacting in person with an individual to assist the individual in achieving objectives to:
(a) acquire, retain or improve self-help skills, socialization skills or adaptive skills that are necessary to for the individual to successfully reside, integrate and participate in the community; and
(b) reinforce a skill taught in school or during the provision of any HCS Program service or non-waiver service.
(2) assisting an individual with his or her personal care activities if the individual cannot perform such activities without assistance.
(3) as determined by an assessment conducted by a registered nurse, providing assistance with medications and the performance of tasks delegated by a registered nurse in accordance with state law and rules, unless a physician has delegated the task as a medical act under Texas Occupations Code Chapter 157, as documented by the physician.
(4) participating in a service planning team meeting.
(5) participating in development of an implementation plan; and
(6) participating in the development of an IPC.
This position involves tasks that may expose the employee to blood, bodily fluids and/or body tissue. All of the job-related tasks may involve potential exposure to mucous membrane or skin contact with blood, bodily tissues, or the potential for spills and/or splashes of these fluids. Use of appropriate protective clothing (Standard Precautions and Blood-Borne Pathogen Procedures) is required for every employed engaged in performing tasks outlined in the definition of day habilitation services.
ESSENTIAL FUNCTIONS
1. Documentation: In-Home ISS Service Delivery Logs, Progress Notes Monitoring Sheets, Behavior Data Collection Sheets, Medication Administration Records, and In-Home ISS Service Training Sheets.
2. Adherence to Universal Precautions and Blood Borne Pathogen Standards.
3. Follow Safety Guidelines.
4. Participation in staff meetings and in service-training.
5. Perform Emergency Evacuation Drills.
6. Responds to emergency needs of individuals including performing CPR, First Aid and performing agency approved Behavior Intervention Techniques as necessary
7. Ensures Functional Independence Skills (FISH) Assessments are completed, documented, and submitted/filed as necessary.
8. Providing training on the individual program objectives.
9. Completes all other duties as assigned.
TASKS THAT REQUIRE EQUIPMENT/TOOLS AND KNOWLEDGE OF THEIR SAFE USE:
1. Measuring vital signs.
2. Weighing individuals using a floor scale.
3. Preparing meals using pots, pans, tableware, utensils, can opener, stove, oven, toaster, food processor, dishwasher, and microwave.
4. Driving to using an automobile or wheelchair van.
5. Recreational activities.
POSITION REQUIREMENTS SKILLS/ABILITIES:
1. Ability to articulate clearly.
2. Must be able to communicate (speak, read, and write) in English.
3. Must be reliable in attendance and timeliness to work schedules.
4. Hearing and vision correctable to normal ranges.
5. Organizational skills.
6. Ability to use body mechanics in controlling unpredictable behavior.
7. Ability to participate in physical activities.
PHYSICAL JOB REQUIREMENTS:
1. Lifting and carrying 40-80 lbs.
2. Pushing wheelchairs of individuals whose total weight may exceed 200 lbs.
3. Operating manual cranks and/or pumps on lifts.
4. Bending, squatting, stooping, and reaching to respond to personal care needs of individuals.
Job Type: Part-time
Pay: From $10.00 per hour
